Medical Value Creation for The Pacey Cuff

The fundamental Value of the Pacey Cuff incontinence device is provided by the preservation of penile blood flow during use. 

      The 100+ year old clamp designs attempted to protect circulation by use of substantial pressure which was moderated by foam cushions. This group of products generally blocks the blood supply when enough pressure is provided to get control of the urethral urine flow.

      The circular cuff designs to date have produced a strangulation effect because of the failure to protect blood flow and thus have pain and limited time of application before relief of the blood supply constriction demands removal.

      The prototyping period of Pacey Cuff development produced a great many devices that had variable success with control of the Urethral urine flow but all of the devices used the fenestrated hood design and the exceptionally effective hood covering the blood vessels of the dorsal area. The devices were applied to a degree necessary to control urine and then each was tested with the hand held Doppler device to locate an arterial signal on the right side and the left side. All of the prototypes passed this test confirming that the blood supply was unaffected. This testing does not however prove that every penis will have adequate circulation because the presence of extensive atherosclerotic vascular disease, previous trauma , or disease like Diabetes Mellitus can have unpredictable local effects.

    The clinical application of the device therefore requires some thought, at times, if expected pain free use is not experienced. The user should therefore  inquire of a Physician or Continence nurse specialist for a second opinion.

The Pacey Cuff was displayed to members of the International Continence Society USA workshop in Phoenix Arizona to good effect.
The clinical need and utility of Pacey Cuff to treat Stress incontinence Post Prostatectomy was confirmed by feedback from attendees .
The Post Prostatectomy management teaching neglects the patient in an important way:

    1. The standard Pelvic Floor Exercise Protocols include referrals to Specialist Physiotherapists who do Pre and Post operative pelvic muscle training for patients . Many argue that the establishment of this training prior to surgery adds a degree of muscular strength that is beneficial post operatively. There would be a logical expectation that this is correct. Clinical Summary

    The Pacey cuff has been used in a post Radiation therapy patient who had Palladium Brachytherapy carried out by Dr Gregory Merrick of Schiffler Hospital in Wheeling , West Virginia in 2001. Then 50 Gray External Beam Radiotherapy was delivered at CCABC using a  Linac linear Accelerator to complete the treatment. The bladder outlet developed calcification and complete obstruction in 2008 when Surgical relief resulted in incontinence.
    Treatment with an AMS 800 device was effective for 2 years and was replaced once. Then the device was removed. The radiation induced vascular changes to the base of the bladder resulted in atrophic changes at the level of the upper urethra.
 
   Pacey Cuff treatment started in early december 2016 and has continued throughout the development period of the device. 24/7 use was practiced throughout the majority of days (275/310 days) . Initial Cystoscopy demonstrated excellent healing of former surgical approaches to the urethra.  The Cystoscopy followup q3months was completed Nov 20 2017 and shows no impact on the Urethra from this intense usage.  The plan to follow this sentinel case will include Cystoscopic examination to be certain that any urethritis will be seen and managed. The current Pacey Cuff has had design changes that make it tissue friendly and the excellent record is likely to continue as more follow up is recorded.

Treating Skin Changes with Pacey Cuff

       The use of Pacey Cuff may, from time , may rarely cause skin irritation that is in part associated with night pad use or soap contact effects as well. The development of any of these changes that are non- infective by nature will be treated primarily with a short period of avoiding Pacey Cuff use. The skin recovery should be prompt and may be assisted by use of a skin protective agend such as Zinc based cream. This type of cream is often used for diaper rash which is also caused by effects of local moisture and provides a moisture barrier for skin.  While Zinc is not recommended treatment for infection , it has been shown to have activity suppressing Staph Aureus which is a common skin resident.
   There are many compounds on the market with zinc that may be selected. An example would be that  referrenced below. Should any rash persist it would be valuable to have a physician consultation or perhaps a telemedicine based consult.
    True infection skin effects will usually require antibiotics but one would expect these changes to be rare with those accustomed to daily bathing and normal activities. Diabetes patients may require some extra vigilance in this regard.

What are the special design features that have made the Pacey Cuff UCD 
exceptional and revolutionary?
  1. The majority of blood vessels supplying the Penis reside in the dorsal or top part of the penile structure . This is shown in the figure supplied. The dorsal arteries and veins reside above the Corpus Cavernosa  which are the spongy venous caverns that provide the erectile function of the penis. The corpus itself is home to a central artery and of course many veins all of which are protected by the hood of the Pacey Cuff Urethral Control Device ( UCD ).                                 
  2.      The fenestrated hood has a large surface area which reduces the pressure exerted by the dorsal component and allows the circulation to continue in and out of the penis. The older clamp devices and the older strangulating cuff designs that have been available for in excess of 100 years cut the circulation at the same time as the urethra is compressed. No design effort was made with these older clamps to provide protection for the circulation.


       3.    The Urethral Compression ( lower component ) has the ability to be 
             deployed at any level  on  the  penis to protect the urethra itself. 
             The lower portion has only  enough pressure to  control  urine flow. 
              This pressure is regulated by the numeric indicators on  the Velcro strap. 
              Thus the device may be applied proximally or distally to vary the 
               point of  application of  Urethral  pressure and at a level of tension known to 
                prevent excessive leakage.

        Extended use of the Pacey Cuff UCD (Urethral Compression Device) may lead to increased length and diameter if the penis. This observation has been made on an individual who has used the device for 10 months during waking hours and at times night. The effect is likely due to the protracted mild stimulation coupled with the effect of preserving the arterial circulation during use. The strangulating clamp designs of the past were unlikely to produce this effect because the arterial obstruction inhibits blood flow and makes the metabolism less active.
