Urinary Incontinence Following Prostate Brachytherapy.


OBJECTIVE:

To define the incidence, time course, and risk factors associated with the
development of urinary incontinence (UI) following prostate brachytherapy.

MATERIALS AND METHODS:

A total of 2461 men were identified who underwent permanent
 interstitial prostate brachytherapy with or without external
beam radiation therapy. We examined the relationship between clinical- and treatment-related
 variables with the onset of UI, defined as leakage requiring pad usage, and further classified
 as stress (SUI) or urge (UUI) predominant, using univariate and Cox proportional hazards
regression models. The changes in International Prostate Symptom Score and quality of life
domains were assessed from baseline to last follow, and examined by UI status.

RESULTS:

Patients were followed for a median of 6.4 years (interquartile range 4.1-9.3).
UI was reported in 108 individuals (4.4%), at a median of 1.8 years
(interquartile range 5 months-4.4 years): 30 with SUI and 78 with UUI. Seventy-two men (66.7%)
 reported using 1, 24 (22.2%) using 2, and 12 (11%) using ≥3 pads per day. On multivariate
analysis, post-implantation transurethral resection of the prostate, urinary retention, 
external beam radiation therapy, and higher pretreatment International Prostate 
Symptom Score were significantly associated with the development of SUI, 
although transurethral resection of the prostate was the only significant risk factor 
associated with SUI. Men experiencing UI reported greater declines in urinary 
quality of life; however, no significant difference was observed between SUI and UUI.

CONCLUSION:

UI occurred in 4.4% of patients following prostate brachytherapy
and is more commonly urge-predominant in character. Distinct risk factors
exist for the development of UUI vs SUI. Urinary leakage requiring pad usage was associated
with declines in urinary QOL.

Patients with Bladder incontinence will understand that we are dealing with a very difficult problem.
Tools that are available are limited so we have created the PaceyCuff System that includes the PaceyCuff and the PaceyCuff guard.
PaceyCuff continues to evolve and upgrade performance due to customer feedback and also ongoing engineering input.
1. Overall strategy- because we are preserving the circulation with our device the traditional clamp practice of complete over pressuring of the Penis is not permitted. The PaceyCuff has a goal to offer great control that is backed up by a pad or ideally the PaceyCuff Guard. This is a combined strategy to cope with this very challenging problem.
2. When the bladder is empty or 1/2 full the pressures are modest and little leakage will be experienced however as filling progresses the stress or pressure incontinence begins to appear. Therefore frequent voiding, when the urge presents itself , assists with keeping dry. 
3. Moderate fluid intake is also a benefit as it keeps the volume in the bladder low.
4. The PaceyCuff Guard gives users good opportunities for an environmentally sound PaceyCuff backup device. The guard has enough adsorbancy to retain drips but if leakage is more of an issue some added absorbant materiel may be added. 

Many PaceyCuff users opt for the PaceyCuff Guard when they purchase and then learn to use the devices together.

     The Guard device is designed to function during the day as well as at night to capture small leaks and provide extra protection to users. If the Guard and Cuff are considered to be parts of a system that is mutually supportive then one would expect that one or more Guards would be useful for each Cuff.
The function of the Guard is enabled by the inner surface which has an adsorptive layer of velour adjacent to the skin. The patent pending Adsorptive* capability is enabled by the monofilament loop field which attracts water or urine by molecular forces alone. These forces can lock down a moderate amount of urine but also release it easily by a simple water rinse or soap and water wash. Drying is best done by compression of the damp device between two layers of towel followed by air dry. The use of a hair dryer on a warm cycle can complete the process in minutes if a rapid turnaround is necessary. 
This device will dramatically reduce the number of pads that end up in the landfill and therefore represents a "Green" solution. Rugged construction assures a long service life. 70% methanol spray or alcohol based aftershave  may be used from time to time to be certain that no bacterial contamination will develop.

*Adsorption:  relies on the molecular attraction of water to the loops of monofilament on a closed cell foam base material.
Absorption:  relies on a material that has open cell structure that allows water to enter into the structure. This is much harder to clean and dry so we do not use these kinds of materials.

       The PaceyCuff depends on the underlying condition of the bladder for most effective use. PMT teaches that users should completely empty the bladder from the most effective standing position wherein the abdominal pressure presses down on the bladder to assist emptying. 
A problem can exist when the bladder outlet scarring , post surgery or radiation,  prevents the complete emptying of the bladder. This can cause retention of a substantial quantity of urine which is easily pushed out of the bladder by coughing , straining, or physical activity like skiing.

      The  urologist can confirm this by cystoscopy but there is a simpler solution for the issue wherein one can do a self catheterization with a 12-14 F size catheter if this is recommended by medical advisors of the patient. The catheterization should be done when the bladder is considered to be empty. If residual urine is present at this time one will know that bladder emptying is incomplete. The process of catheterization also tends to solve the problem by dilating the bladder outlet.

       How to do Kegel Exercises

  • The use of Pacey Cuff during Kegel Exercises will prevent unwanted urine leakage during exercise . 
  • Use the Pacey Cuff with an empty bladder.
  • Discuss the plan with your Urologist or Continence Nurse expert.
  • Keep a log of your progress.

  1. Find the right pelvic floor muscles. To decide which muscles to contract , stop urination midstream. The muscles that were used for this are the muscles that you need to strengthen for control of the leakage
  2. Tightly contract these muscles for 7-10 seconds without tightening your buttocks, abdomen, or thigh muscles. If you contract your pelvic floor muscles while looking into the mirror , the base of your penis will move closer to your abdomen and your testicles will lift. This should feel like a lifting motion. Then relax for 7-10 seconds. This is 1 repetition of the exercise.
  3. Try to do about 45-60 Kegel repetitions daily , at one session or split sessions.
  4. These exercises may be done standing, sitting, or lying down and while you're enjoying other activities, such as watching TV or reading a book.
