       How to do Kegel Exercises

  • The use of Pacey Cuff during Kegel Exercises will prevent unwanted urine leakage during exercise . 
  • Use the Pacey Cuff with an empty bladder.
  • Discuss the plan with your Urologist or Continence Nurse expert.
  • Keep a log of your progress.

  1. Find the right pelvic floor muscles. To decide which muscles to contract , stop urination midstream. The muscles that were used for this are the muscles that you need to strengthen for control of the leakage
  2. Tightly contract these muscles for 7-10 seconds without tightening your buttocks, abdomen, or thigh muscles. If you contract your pelvic floor muscles while looking into the mirror , the base of your penis will move closer to your abdomen and your testicles will lift. This should feel like a lifting motion. Then relax for 7-10 seconds. This is 1 repetition of the exercise.
  3. Try to do about 45-60 Kegel repetitions daily , at one session or split sessions.
  4. These exercises may be done standing, sitting, or lying down and while you're enjoying other activities, such as watching TV or reading a book.

Research News:

The most important contribution made by the invention of the Pacey Cuff male continence device is that the circulation is preserved when the cuff is applied at pressures that are customary for Urethral compression control of urine flow. The development of the cuff was guided by the measurement of blood flow to the glans by use of a hand held digital doppler vascular flow ultrasound.  The doppler signal was compared with Classic Clamp designs and found to be variable in older designs that typically have ischemic pain which limits comfort . The Pacey Cuff has consistent pulses with all variants tested during the development and this was particularly so with the low compression Night Cap design of 2018. This variant is designed for night use in combination with an awakening device for extra safety when users take medication to sleep or for pain.

Pulse Oximetry Experiments with the Pacey Cuff

PMT has had the benefit of an engineering co-op 4th year student from the UBC Engineering Physics program during the summer of 2018. Matt Hill , our student, worked with PMT engineering staff Ian Mcdougall and Marc Alphonso of PLC electronics. This has given us many benefits including the development of a testable Pulse Oximetry variant of the Pacey Cuff Turbo device. A very science intensive summer! We also developed the Night Cap device and other products for introduction to the market this fall.

Results:

Conclusion:
         The Pacey Cuff Urethral Control device does provide excellent protection of the circulation in              the penis when normal vascular function and anatomy is present. This confirms the truth of the                  PMT claim that the circulation is protected.

Incontinence for those with Uncircumcised Penis

Many men have not been circumcised and therefore have issues that accompany this part of the anatomy. The US population is roughly 80 % circumcised whereas only 30 % of young Australian males are circumcised at birth. There is a trend to discontinue the practice taught for centuries to be necessary for cleanliness and health . The trend to fewer circumcisions is well established.

Pathology associated with the foreskin:
  • viral infections (genital warts, genital herpes simplex)
  • bacterial infections (syphilis, chancroid)


  • infestations (scabies, pediculosis)
  • skin disorders (lichen planus, psoriasis, contact dermatitis, fixed drug eruptions)
  • balanitis (plasma cell infiltration, Reiter's disease)
  • precancerous conditions (giant genital warts, Bowen's disease, erythroplasia, Bowenoid papulosis)
  • penile cancer (squamous carcinoma)

PaceyCuff.com  may be used with those who have an intact foreskin if one is aware of the special considerations that must be addressed. The Pacey Cuff when applied to the shaft of the Penis has a clearly defined setting that provides control of the urine. Also if the foreskin is retracted on  application it will extend to a normal position and carry the Pacey Cuff off of the end of the shaft.
One user reports a practice of pulling the foreskin down toward the tip before application of the Pacey Cuff. This makes sense because the redundant skin will not be bulky but will be thin and therefore accurate and consistent numbers will be obtained.



  • Void to get the bladder empty before you start.
  • Keep the bladder between empty and 1/2 full so that bladder pressures are moderated. This is done by a combination of moderating intake and voiding frequently.
  • Extend the foreskin to cover the Glans penis.
  • Apply the Pacey cuff and squeeze the upper part and the lower part together  with the dominent hand while avoiding skin pinching between the two parts. If skin is in the gap it should be removed at this time to allow closure.
  • Note the number achieved (3-4 perhaps )and then check to clear any pinching to avoid discomfort.
  • When the number is achieved that prevents 99% of the leakage ,note this number for future reference because you will need to set it for future use .
  • The bottom part can also be adjusted if required .

A top complaint: Nearly two-thirds of respondents reported dissatisfaction with the sex lives, and 53% had sexual dysfunction. Roughly a quarter had erectile dysfunction. These symptoms often were associated with no longer “feeling like a man” and fears of partner abandonment. Other troubling symptoms included fatigue (20%) and overactive bladder (19%). Half of patients were bothered by treatment side effects, and 34% had pain that prevented enjoyable activities.
With regard to mental health, 62% expressed sadness and 68% worry that their cancer could progress. By stage, 35% of patients had been diagnosed with stage 1, 31% stage 2, and 33% stage 3 or 4 PCa. (Another 12% of men did not remember their diagnoses.) Seven in 10 men underwent follow-up tests and scans twice a year or more. For 1 in 5 patients, PCa was not their sole malignancy.
Overall, three-quarters of patients reported satisfaction with their medical care, but lacked sufficient mental health resources and treatment. Many men wanted support in combating depression and anxiety. One in 10 patients were eventually diagnosed with anxiety or panic disorders (9%) or a mood disorder (8%), such as depression.
Emotional upset often persisted. Although 40% were in remission, 14% had at least a single recurrence. If they had to sum up PCa in one word, men chose “scary,” “life-changing,” and “manageable,” among other terms. One respondent said that, while breast cancer survival is often considered a badge of honor, “men who survive PCa wear it as a badge of shame.”
The survey coincided with the launch of ProstateCancer.net, a site dedicated to supporting, educating, and connecting people affected by PCa.

1. The Goal of use of the Pacey Cuff is to apply correct pressure levels to the Urethra on the bottom surface of the penis. If the bottom part is extended too far one cannot tighten the device properly.

The 2 trials will be reported this week at the Genitourinary Cancer Symposium 2018 in San Francisco where it is expected these drugs will propel a major change in therapy.  

Both trials were done on men post treatment who had PSA double within 10 months time span indicating active cancer cell development.
The  unanswered question of which drug is superior must remain conjecture until a proper head to head comparison is carried out.

Exercise alone and various combinations of interventions reduces the risk of injurious falls in older adults

Exercise alone; exercise combined with vision assessment/treatment; exercise combined with vision assessment/treatment and environmental assessment/modification; and clinic-level quality improvement strategies combined with multifactorial assessment/treatment and calcium and vitamin D supplementation are all effective interventions for reducing the risk of injurious falls in older adults.

Synopsis
These investigators thoroughly searched multiple databases including MEDLINE, EMBASE, the Cochrane Register, Ageline, and reference lists of relevant trials and reviews for randomized controlled trials that examined fall-prevention interventions for adults 65 years or older. Study authors were also contacted for unpublished studies or additional data. Two investigators independently reviewed all potential studies for inclusion criteria and methodologic quality using standard risk-of-bias scoring tools. Conflicts were resolved by consensus agreement with a third reviewer. The primary outcome of interest was the number of injurious falls and fall-related hospitalizations. A total of 283 randomized trials and 20 companion reports (N = 159,910 participants) met inclusion criteria. The overall risk of bias among the studies was moderate, with an unclear risk of bias for allocation concealment, contamination, and selective outcome reporting. A funnel plot analysis found no evidence of publication bias. 

Conclusions:

   1.  Four interventions were significantly associated with a reduced risk of injurious falls compared with usual care: exercise alone; combined exercise and vision assessment and treatment; combined exercise, vision assessment and treatment, and environmental assessment and modification; and combined clinic-level quality improvement strategies, multifactorial assessment and treatment, calcium supplementation and vitamin D supplementation. 

2.  Combined exercise and vision assessment and treatment was the most effective intervention. In a subgroup analysis, the best intervention for reducing the risk of hip fracture was combined osteoporosis treatment, calcium supplementation, and vitamin D supplementation.
